Where behavioral health billing gets complicated
Behavioral health claims follow different rules than most medical billing. These are the areas we see practices struggle with most:
Testing codes and units
Psychological and neuropsychological testing is billed with separate evaluation and test-administration codes (such as the 96130–96139 series), with time-based units that must match documentation. We review selected units against the recorded work.
Prior authorizations
Many payers require authorization before testing or ongoing therapy, and authorized units need to be tracked against what's billed. We track authorization status alongside claims.
Time-based psychotherapy codes
Session codes like 90832, 90834, and 90837 depend on documented time, and add-on codes have their own requirements. We review the record against the selected codes.
Behavioral health carve-outs
Some health plans route behavioral health claims to a separate behavioral health administrator with its own rules and portals. The administrator and claim submission path need to be identified.
